•Delivery Site:
location where the primary care services are delivered. Note:a grantee or main organization could have more than one delivery site.

•Main organization:
Grantee site location.Part of the Consolidated Health Center Program.

•Consolidated Health Center Program:
Combines Community Health Centers, School Based Programs, Health Care for the Homeless Programs, Migrant Health Programs, andPublic Housing Primary Care Programs entities.  In Puerto Rico there is not a Public Housing Primary Care Program.The basic funding for health centers is awarded under section 330 of the Public Health Service Act.

•Health Care for the Homeless Grantees(HCH) (section 330(h):
Provide services to homeless individuals and families within the service area.

•School Based Health Centers (SBHCs):
Community-based health care providers collaborate with school systems to establish and implement SBHCs that provide comprehensive primary and preventive health services.
